1. FamilyMedApp is a record-keeping tool

FamilyMedApp is a consumer software product that helps individuals and families store, organize, and retrieve their own health information — such as appointments, medications, conditions, and vital-sign readings. The Service is a personal record-keeping tool only.

2. Not medical advice

Nothing shown in or produced by the Service, including any text, calculations, reminders, charts, notifications, exports, summaries, or other output, is medical advice, diagnosis, or treatment. Content is based solely on information you enter and on standard display logic. It has not been reviewed or endorsed by a licensed healthcare professional for your particular circumstances.

3. Not a substitute for professional care

Always seek the advice of a qualified healthcare professional — such as a physician, nurse practitioner, pharmacist, or other licensed provider — with any questions you may have about a medical condition, medication, symptom, or treatment. Never disregard, avoid, or delay seeking professional medical advice because of something you read, recorded, or saw in FamilyMedApp.

4. Not a medical device

FamilyMedApp is not a medical device and has not been evaluated, cleared, or approved by the U.S. Food and Drug Administration, the European Medicines Agency, Health Canada, the UK MHRA, or any other regulatory authority. The Service is not intended to diagnose, treat, cure, mitigate, or prevent any disease or health condition. It must not be used as the basis for making clinical decisions.

5. Not an electronic health record (EHR/EMR)

FamilyMedApp is not a certified electronic health record system, a hospital system, an insurance claims system, or a pharmacy management system. Do not rely on the Service as the official or complete medical record for you or anyone in your care. Always keep an independent copy of important medical information and share it with your healthcare providers directly.

6. Accuracy of information is your responsibility

The content of your records depends entirely on what you enter. FamilyMedApp does not verify, validate, correct, or complete the information you provide. Errors or omissions in what you enter may lead to incomplete or incorrect displays. Review your entries carefully and cross-check them with authoritative sources such as prescription labels, discharge summaries, and your healthcare providers.

7. Reminders, notifications, and interaction warnings

Any reminders or notifications the Service displays (for example, medication reminders or appointment alerts) are provided as a convenience. They may fail to appear due to device settings, network issues, browser behavior, operating-system updates, or other factors beyond our control. Do not rely on FamilyMedApp reminders as your sole or primary means of ensuring that medications are taken or appointments are kept. If drug-interaction, allergy, or similar warnings are ever shown, they are general, non-exhaustive informational aids and are not a substitute for review by a licensed pharmacist or physician.

8. No provider-patient relationship

Your use of FamilyMedApp does not create a physician-patient, pharmacist-patient, nurse-patient, or other provider-patient relationship between you and FamilyMedApp LLC, its members, employees, or contractors.

9. Third-party information

The Service may contain or link to information from third parties (for example, calendar services you choose to subscribe to). FamilyMedApp does not endorse, and is not responsible for, the accuracy, completeness, or timeliness of any third-party content.

10. Limitation of liability

To the fullest extent permitted by law, FamilyMedApp LLC and its officers, members, employees, and agents disclaim all liability for any decision you make, or refrain from making, in reliance on information stored in or displayed by the Service.
